Medical Forms for Returning Students
- Do NOT need to be updated unless instructed by the preschool office
- All parents will be notified when the medical form on file needs to be updated by a physician
Information Needed
- All Spaces on all forms need to be filled out (NO Spaces)
- If something does not apply, N/A or line in the space is acceptable

- Information Card - For teachers to carry
- Child Enrollment & Health Information - Required by the State of Ohio for our license - ODJFS #01234
- Child Medical Statement - Required by the State of Ohio for our license - ODJFS #01234
- Family Information - For teachers to review
- REMIND - Text system used for communicating emergency closings

How to Submit
- Email - powellumcpreschool@gmail.com
- Hand deliver to Church Building (East entrance) open Monday - Thursday: 9-3. Place in clear box located on the counter at church Welcome Center or hand it to a Church or Preschool Office staff member
- Postal Service: PUMC Preschool - 825 E. Olentangy St. Powell, Ohio 43065
Over the summer you can expect
- A reminder email to submit forms (early June)
- An email regarding Orientations (end of July)
- A letter from the teachers regarding classroom assignment and orientation
- Other miscellaneous emails
