    • To enroll at PUMC Preschool for the 2026 - 2027 school year, I agree to pay the NON REFUNDABLE Registration Fee.
    • To hold my child’s spot for the 2026–2027 school year, I agree to pay a deposit equal to one month’s tuition by May 5, 2026. The deposit will be applied to my child’s last month of tuition or refunded with a 30-day written notice. Failure to pay the deposit will result in forfeiture of my child’s spot at PUMC Preschool.
    • Tuition is an annual fee, payable by cash, online, or with a check to PUMC Preschool. You may choose to pay all at once or in 9 monthly installments, which are due by the 1st day of each month beginning September 1, 2026 and ending May 1, 2027. Families who wish to pay tuition in full for the school year may do so by September 1, 2026 and will receive a 5% discount. Payment must be made in one lump sum and is subject to the school’s refund and withdrawal policies.
    • Payments made online (either monthly or in full) will be subject to a processing fee. This fee is required as part of your payment.
    • A late fee of $25.00 will be charged when any payment is received on and after the 6th of the month.
    • An insufficient funds fee of $25.00 will be charged if a check is returned by the bank.
    • Full tuition for each month is required regardless of holidays, vacations, illness, school emergency closings, or withdrawal.
    • A one-time annual supply fee is due with your child’s first month of tuition.
    • I acknowledge my child is immunized in accordance with the requirements of section 5104.014 of the Ohio Revised Code.
    • I give permission to PUMC Preschool to use photos of my child in the class book, website, social media and/or other formats for the public.
    • I give permission to PUMC Preschool to share our name, address, phone & email to other parents, staff & preschool board based on my preferences stated on the Child Enrollment and Health Information for Child Care form (JFS 01234).
    • **WITHDRAWAL – School Initiated – The school reserves the right, in extreme circumstances, to terminate a child’s enrollment if, in the opinion of the staff, continued attendance is not in the best interest of the child or the school; if payments are not kept current; or if required forms are not provided. I understand that the registration fee is nonrefundable.
    • **WITHDRAWAL – Parent Initiated – I agree to notify the preschool in writing as soon as possible if my child will not be completing the school year. I understand that a 30-day written notice is required in order to receive my deposit back. If I withdraw my child in March, April, or May, I agree to be responsible for tuition for the remainder of the school year. I understand that the registration fee is nonrefundable.
